When prescribed by a physician, we provide the following services to Barry, Branch, Calhoun, Kalamazoo and Van Buren counties:

Intermittent Nursing Services

Home Health Aide Services

Medical Social Work Service

Physical Therapy

Occupational Therapy

Our skilled professional services are geared for individuals who may have been recently hospitalized or are in danger of hospitalization or nursing home placement. We offer services to individuals of any age.

A registered nurse does the initial assessment that includes blood pressure, heart rate and weight check, pain assessment and medication review. In addition a general medical assessment including eyes, ears, nose, month, mental state, neurological, muscular and cardiovascular and respiratory status.

In addition to the above, persons who meet Medicare/Medicaid or Blue Cross and Blue Shield criteria for home health care will have the following assessments performed:

Comprehensive Safety Assessment

Comprehensive Assessment of Activities of Daily Living, including grooming, feeding, etc.

Health Risk Factor Assessment

Caregiver Assessment

Teaching/Learning Assessment Related to Self-Care

If you meet this criteria for home health care services, the nurse will begin the process for admission into home health care skilled nursing services. Many home care services are covered by insurance. We can contact your insurance carrier to determine coverage and bill them directly to save you the trouble of dealing with paperwork.
